Care Planning Resources

The RCGP recently launched their Collaborative Care and Support Planning programme with a short, engaging animation which describes the care planning process and the impacts it can have, with carers as a part of this.

The Collaborative Care and Support Planning programme is a three year programme, funded by the Health Foundation, which aims to embed person-centred care and the collaborative care and support planning process as a core part of the way general practice works with people with long term conditions. This approach has the potential to reduce the workload burden on GPs and practice teams, and improve outcomes for the person with the condition(s).
